Step 1: Login

Step 2: Download Form

• Tap Download Form

• Select form by clicking checkbox

• Tap Get Selected

Step 3: Start Survey

Tap Start New Form to start the survey. After completing the survey click Save Form and Exit.

Note: - Swipe right for next question/screen and left for previous.

Step 4: Drafts

• See all the forms saved as draft as well as finalized

• Choose the form you want to edit

• Save form as finalized

Step 5: Send final form on server

• Tap Ready to send

• Select form by clicking checkbox

• Tap Send selected

Steps for changing language

1. Tap three dots from right corner

2. Tap change language

3. Choose the language you want

Sent: You can see the count of forms that had been sent to server by you.

Delete form: If you want to delete the form

• Click Delete form button

• Enter your password (same password which you entered at login time) and click ok

• Choose the forms you want to delete from SAVED FORMS & BLANK FORMS corresponding screens.

• Click “Delete Selected”

For downloading and sending form, internet connection is required.
